Care records and care pathways

Indhold

All the work around each person, gathered in one place

Profile, contacts, calendar and attendance, all held on the citizen. Every relevant piece of information sits in one place, so a new colleague can find it without having to ask.

  • One profile per citizen
  • Relatives and guardian in place
  • Attendance and appointments together

Why it makes the daily work easier

Information is where you look for it

See the citizen's profile quickly and easily, with all the relevant information gathered in one place and in the same order for every citizen.

It is clear who may be contacted

Relatives, guardian and caseworker are stored as contacts with a role and notes, so nobody has to guess who they are allowed to call.

Appointments do not vanish into a calendar nobody sees

The citizen's calendar belongs to the record, so appointments, visits and transport sit where the work happens.

Confidentially on to a caseworker, straight from the record

Information about a citizen sometimes has to leave the organisation, to a caseworker or a relative. That should happen from the record, not from a private email account.

The contacts are stored on the citizen, so the message goes to the right person. It is locked with a password, and both the message and the reply stay on the record.

What's inside

A citizen in CitizenOne is more than a profile. The record has 19 tabs in total, and each service shows only the ones it actually uses. Journal, plans, documents and finances are tabs on the citizen, and that is the whole point: the work is gathered around the person rather than spread across several systems.

Profile

  • Citizen profile: all the relevant information about the citizen gathered in one place.
  • Tabs as needed: show only the tabs your service actually uses.
  • Archiving: archive and reopen citizens without losing their history.

Contacts

  • Relatives and guardian: register contacts with role, consent and notes.
  • Send to a contact: send a message to a contact directly from the citizen's record.

Everyday work

  • Citizen calendar: appointments, visits and transport on the citizen's own calendar.
  • Attendance: register attendance and absence at your service.
  • Allocated hours: follow the hours allocated to the citizen.

Gathered around the citizen

  • Journal: the citizen's notes live on the citizen, not in a separate archive.
  • Plans and goals: support plan, sub-goals and follow-up on the same record as the journal.
  • Documents: the citizen's own folder, with the files that belong to the record.
  • Finances: expenses and pocket money on the citizen, kept separate from the organisation's finances.
  • Outcome measures: record whether the support is working, on the citizen it concerns.
  • Timeline: the whole care pathway in one chronology, across the tabs.
  • Relations: relatives, guardian, network and children, each with the access they should have.

Security and access

  • Access by department and role: staff see the citizens their role and department give them access to (GDPR Article 32(4)).
  • Consent can be documented: consent forms are registered on the citizen, so you can show what consent has been given for (GDPR Article 7).

Works with the rest of CitizenOne

  • CitizenOne Journaling: the profile is the entry point to the citizen's journal and plans.
  • CitizenOne Secure email: write confidentially to a caseworker or relative from the record.

Who it's for

New colleague

You need an overview of a citizen before your first shift and do not know where to look.

The profile has the same structure for every citizen, so the information can be found without asking.

Children's home

Parents, school and caseworker have different roles, and it is unclear who may do what.

Contacts have roles and notes, so it is clear who can be contacted about what.

Frequently asked questions

Can we hide the tabs we do not use?

Yes. The tabs on the citizen follow the modules and permissions your service has taken into use.

What happens when a citizen moves on?

The citizen can be archived, and the history is kept. If the citizen becomes relevant again, the record can be reopened.

Can we see who has read a citizen's information?

Yes. Reading and changes are logged, so it can be documented in connection with an access request or an inspection.
